2013 Insurance Cards

A new plan year may mean new insurance cards. Check the list and see if you should receive new membership cards for 2013.

CareFirst BlueCross BlueShield (medical & dental), EHP Classic & BlueChoice HMO:
All participants will be receiving new enrollment cards at the beginning of January, 2013.

Kaiser Permanente HMO & United Concordia Dental:
Cards will only be sent to new participants and existing members who made changes.

Cigna Dental:
Membership cards are not issued. The JHU Plan ID# is 3215300.

MEDCO/Express Scripts Prescription:
Cards will be sent to new participants. Also, employees who switched medical plans will receive new cards identified with the new group number. Employees who made changes within the same medical plan will not receive a new prescription card and can continue to use their current card. Prescription cards are also accessible at any time on the MEDCO/Express Scripts website at www.express-scripts.com if you register online.

United HealthCare (Voluntary Vision Plan):
Cards will be sent to new participants only. Current participants who made changes can continue to use their existing card. Membership cards will also be accessible on the United Healthcare website at www.myuhcvision.com as of January 1, 2012. Complete the information requested including your Social Security number as your unique identifier. Once on the site, you will see the link to print your ID card in the top right.

WageWorks Health Care FSA Reimbursement Card:
Cards will be sent to new participants and existing members whose cards have expired. The expiration date is noted on all cards.

Insurance cards are mailed to members from the provider. Contact the provider directly to check on the status of your cards or to request additional cards. Phone numbers for all benefit plan vendors are available on the Benefits website.
