2015 Benefits Confirmation Statements Mailed

Confirmation statements for 2015 benefits have been mailed to all employees who were actively enrolled in our plans as of 12/1/2014.

Please note that if you are covering a child who is currently age 26; coverage for them will end on 12/31/2014 and is reflected as such on the confirmation statement.

If you determine after receiving your confirmation that your enrollment is not correct, you must act on this no later than 12/31/2014 by:

  • Emailing the university Benefits Service Center at with the correction(s).
  • Entering “2015 Annual Enrollment Correction” in the subject line.
  • You will receive a return email acknowledging receipt of your request.
  • A corrected confirmation statement will be mailed to you in January.
